Did you know free income tax help is available all around Polk County? If your family income is less than $66,000, you are eligible to have your taxes prepared for free! There are both walk-in and appointment only sites.

 

You will need the following to complete your taxes:

CHECKLIST

​

• Your email address and password *

• Photo ID for you and your spouse *

• Social Security Cards for you, your spouse, and dependents *

• Birth dates for you, your spouse, and dependents

• Form W-2, W-2G, and/or 1099-R from all employers

• Interest and dividend statements from banks (Forms 1099)

• A copy of last year’s federal and state returns, if available (Your PIN and AGI)

• Bank routing; savings & checking numbers (for use with form 8888) for Direct Deposit

• Childcare Provider’s Name, address and Tax ID number
